There are an increasing number of healthcare professionals in place to support the work of the GP for patients in the community.
District Nurses – who provide nursing care for those who are disabled, palliative or recently discharged from hospital.
Community Matrons – who maintain a close eye on those housebound patients with multiple medical conditions and who may be at risk of frequent hospital admissions.
Macmillan Nurses – who provide end of life care and support to those patients whose needs are palliative.
Health Visitors – who are mainly concerned with the care and protection of those aged under 5.
