Please note we are only currently able to offer travel vaccines that are available on the NHS. For non-NHS vaccines you will be advised to attend a ‘travel clinic’ to obtain private vaccines for which there will be a cost to you.

Please complete the Travel Health Form to enable our nurses to review your travel arrangements: Document Version (Patient Travel Health Questionnaire) and return to the surgery either by email, post or hand. This should be at least 6 weeks before you are due to travel.
Once the form has been reviewed you will be contacted to attend for an appointment to administer the vaccines and given advice on attending a travel clinic for the possibility of requiring additional private vaccines before travel.
