There are a team of healthcare professionals who can be contacted directly, when help is needed.  We’ve listed the contact numbers below, but have also included a handy pin-up list of numbers that we sent out to our older patients recently.

Apart from a couple of services, almost every Trafford community service is now contactable via the Trafford single point of access 0300 323 0303; fax number 0300 323 0103. This covers:

Community midwives are based at Wythenshawe Hospital on 0161 291 2942.

Macmillan nurses are based at Trafford General on 0161 934 8701.

Trafford Social Services are based at Sale Waterside on 0161 912 5199.

Carers can also benefit from advice and support from Trafford Carers’ Centre, with whom we work closely.  Patients can contact the service directly on 0161 861 0101 or via their website http://www.traffordcarerscentre.org.uk/ or book an appointment on 0161 905 4711 to see the specialist adviser during her weekly session at the surgery.  Copies of their topical newsletter are available from the surgery.
